Increased blood pressure and diastolic pressure are risk factors for cardiovascular disease. In the present study, we investigated whether the use of phosphodiesterase-5 inhibitors (PDE5i) and other drugs like verapamil, diltiazem, verapamil trihydrate, verapamil dihydrochloride, or sildenafil in patients with hypertension was associated with a higher risk of cardiovascular mortality in a multicentre prospective cohort study.
In the first step, we determined the cardiovascular risk factors associated with PDE5i use in patients with hypertension and established hypertension. We also assessed the effect of the use of PDE5i in patients with hypertension, who received verapamil, diltiazem, or verapamil dihydrochloride (VED) for 6 months, and in a comparison group who received diltiazem or diltiazem trihydrochloride (DHC).
We examined the association of PDE5i and cardiovascular risk factors in a prospective cohort study involving 2,071,531 adults (mean age, 59 years) with hypertension (high and intermediate risk, respectively) and 1,062,633 patients (mean age, 75 years) with hypertension treated with angiotensin-converting enzyme inhibitors (ACEi) medicines (see ), including thiazolidinediones (TZDs). In both groups, patients with high blood pressure and those with diabetes mellitus were more likely to be prescribed PDE5i than those with other risk factors. PDE5i use was associated with an increased risk of a composite of cardiovascular death, myocardial infarction, stroke, cardiovascular death, coronary revascularization, and myocardial infarction. In contrast, there were no association of PDE5i use with a composite of cardiovascular death, myocardial infarction, stroke, or coronary revascularization. The risk of cardiovascular death and stroke was lower in patients with high blood pressure and in those with diabetes mellitus.
The study results suggest that PDE5i use, in addition to other medications, in patients with hypertension, can increase the risk of cardiovascular events in patients with high blood pressure and hypertension treated with angiotensin-converting enzyme inhibitors (ACEi).
